How to Submit a Case
Click on the 'Submit a Case' button at the top of the page then select the type of case you would like to submit. Complete the prescription and upload patient photos. The case photos required are:

  1. Relaxed Smile and/or Exaggerated Smile
  2. Retracted Slightly Apart
  3. Anterior Occlusion
  4. Right Occlusion
  5. Left Occlusion
  6. Upper Occlusal
  7. Lower Occlusal
After completing the prescription, you will have the option of either mailing PVS impressions to Six Month Smiles or you can upload your Digital Impressions. If selecting PVS impressions, a shipping label will appear after you enter in the payment details. Print the shipping label and any other necessary documentation to mail the case to Six Month Smiles.
 
You will be notified via email when your treatment plan is available and ready to review and approve. You can also log into the PRC and check to see if a treatment plan is available.
