Each Provider decides their cost for Six Month Smiles treatment. A typical Six Month Smiles treatment costs between $3,500 and $5,000. If you have a Six Month Smiles Provider and know what they charge, then apply for that specific amount. If you have not found a Provider, $5,000 is a recommended amount to apply for. Submit a Request A Consultation form and our Patient Advocate will locate a provider near you. 

You will be contacted by our Patient Advocate who will refer you to a Six Month Smiles Provider near you. The Doctor will contact you to set up a consultation. In the meantime, you can look up a Provider near you.

You can apply for a provider's treatment quote at this page, without needing to leave the office. Click "Get Started" to begin your application on the LendingClub Patient Solutions website. You will discover your options in minutes, allowing you to begin treatment promptly.

You will receive a letter from LendingClub Patient Solutions' bank partner regarding the status of your application. If a friend or family member is applying on your behalf, have them indicate you as the patient on the application. If you don't prequalify, contact our Patient Advocate for help finding a provider near you. Providers may have other payment and financing options available.

Your reference number is located on the approval email you will receive from LendingClub Patient Solutions after you have been approved. This is called your Application ID.

After you have been prequalified and completed your consultation, select a financing option with LendingClub Patient Solutions. You or your provider will need to fill out this form to verify your information and final treatment cost. Once our Patient Advocate confirms the amount, you will receive an email from LendingClub Patient Solutions detailing how you can obtain and complete your financing documents.
