Are you ready to add Six Month Smiles to your practice?
Six Month Smiles is designated as an Approved PACE Program Provider by the Academy of General Dentistry. The formal continuing dental education programs of this program provider are accepted by the AGD for Fellowship, Mastership and membership maintenance credit. Approval does not imply acceptance by a state or provincial board of dentistry or AGD endorsement. The current term of approval extends from 8/1/2012 to 7/31/19. Provider ID: 341087
Course Cancellation Policy: If the cancellation is received 2 weeks prior to the course beginning, you will receive a refund minus $1,000. The $1,000 credit can be applied to a future course within 1 year. If you cancel less than 2 weeks prior to the start of the course, no refund, credit or transfer will be issued.
On rare occasions, air traffic delays or poor weather could affect the start time of a seminar or force us to cancel the seminar. If we are able to start course by 1PM of the first day (Friday) there is no refund … all topics will be covered in full. If we are not able to hit the 1PM deadline the course will be condensed into a one day course to be held on Saturday and you will receive a $1,000 refund. If both days of the course are cancelled your tuition can be applied to a future course or you may opt to receive a full refund.
Transfer Policy: You can transfer your course date/location up until 2 weeks prior to the first day of the course. No transfers will be accepted less than 2 weeks prior to the start of a course. Failure to utilize the transfer, within one year from the original transfer date, will result in the loss of all funds.
